According to the announcement of the Chancellery of the Prime Minister on the organisation of vaccinations against COVID-19 in workplaces, I would like to inform that the Silesian University of Technology opens registration for vaccinations for employees, associates (employed under civil law contracts or self-employed), doctoral students and students over 18 years of age, as well as family members of the above mentioned persons.

Registration at the Silesian University of Technology will be conducted until 3 May 2021 till 2 p.m. The vaccination for the Silesian University of Technology is carried out by the Municipal Hospital No 4 in Gliwice - Vaccination Centre at the "TAFLA" ice rink, located at ul. Akademicka 29 in Gliwice.

HOW TO REGISTER FOR VACCINATION

Persons who wish to participate in vaccination against COVID-19, and wishing to register their family members for vaccination, should register via web form available at https://e-politechnika.polsl.pl/index.php/application-form-for-vaccination-login/
