WE ENCOURAGE YOU TO FILL OUT THE FORMS ELECTRONICALLY
After completing the form, within 7 working days you will receive a PDF file by email to print out yourself (double-sided). Please sign it and send it immediately to the Project Office (or to the WIB reception desk). The Application Form should be completed ONLY ONCE. After successfully completing the recruitment process, you can apply to participate in the selected form of support.
DO NOT STAPLE THE PAGES, PRINT ON BOTH SIDES
The candidate will receive information about their qualification for participation in the project at the email address provided.
Alternatively, printed and completed documents (Project Participation Declaration, Personal Questionnaire, and Initial Competency Test) should be delivered to the Project Office or the drop box at the reception desk of the Department of Biomedical Engineering.
