KIR electronic signature qualified certificates
The ordering of a qualified certificate is based on the letter from the Vice-Chancellor for Science and Development on electronic signatures dated 4 July 2018. - it contains a model order form.
The following is the only way to request a qualified certificate:
- Download the editable form as a PDF file: KIR_Form.pdf
- Open the form, if in doubt please use Adobe Acrobat Reader.
- Please complete the editable fields of the form as instructed: KIR_Manual.pdf
- After entering the data, please save your changes.
For Acrobat Reader, this happens when you try to close the file or the program itself. A dialog should then appear asking if you want to save changes for this file, please confirm and save the file. - Save the form as an uneditable document.
For Acrobat Reader, this is done by opening the Print window (Ctrl+P), selecting Print to PDF from the Printers list and finally selecting the Print button. - Ensure that the document name has the form "KIR_surname.pdf", e.g: KIR_Kowalska.pdf
- Please confirm that the information is correct and true.
- Send the created uneditable document file by e-mail to the IT Centre secretariat at the following address: CI@polsl.pl
- When sending the form to CI@polsl.pl, specify the financial dimensions of the unit for which the debit note is to be issued in the body of the message.
The application document will be signed electronically by the Vice-Chancellor for Science and Development and the Quaestor. Please do not attempt to obtain their signatures or stamps yourself.
If the uploaded file contains elements not foreseen in the above procedure, it will not be considered as a valid application and the applicant will be asked to correct it.
Responsibility for the information contained in the document lies with the person submitting the document to the IT Centre. Please remember that an electronic signature is a form of a state document and is subject to relevant legal regulations.

The approximate time for issuing the certificate is about 10-14 days after sending the completed form to the IT Centre. The employee will be informed about the possibility of collection by phone or e-mail.
In the case of individual orders, collection is possible at the branch of Krajowa Izba Rozliczeniowa in Katowice during business hours. Appear in person with the document whose series and number are indicated on the form.
Due to the global virus threat, until further notice, the only way to receive a qualified certificate will be in person at the office of the National Clearing House in Katowice.

Note
From 1 January 2025, the IT Centre is obliged to issue debit notes for units for the purchase or renewal of a qualified certificate.
When sending the form to CI@polsl.pl, specify the financial dimensions of the unit for which the debit note is to be issued in the body of the message.
