When you are EU/EFTA/the Swiss Confederation citizen:

You have the right to entry the Republic of Poland without visa. You can stay on the territory of Poland up to 3 months on the basis of valid passport or identity card. After the period of 3 months you are obliged to register your stay.

Remember that it can be done no later than on the next day following that period of time.

In order to do it you are obliged to:

  1. fill in the application for registering the residence which can be found here

It has to be filled in Polish (1 application form and 3 copies) and submitted in person to Silesian Voivodship Office in Katowice.

  1. submit valid travel document (passport) or other document confirming identity and citizenship (2 copies and an original for the purpose of presenting it to the clerk)
  2. submit 4 recent biometric photographs (taken not earlier than 6 months before the date of application)

If the applicant has been given a PESEL number and is registered, the application must be attached with current documents confirming this (certificates from the city or municipal office).

  1. submit a certificate of admission or continuation of studies issued by the faculty office, or a certificate of participating in a vocational training
  2. a document confirming health insurance coverage or possession of private health insurance
  3. submit a declaration on having sufficient financial resources to support oneself in the territory of Poland. It can be a a certificate of the limit on the credit card issued by the bank that issued the credit card, a certificate of the possession of payment funds in a bank located in the territory of the Republic of Poland or document confirming the granting of a domestic or foreign scholarship
